Support at home and in the community
We support adults with intellectual and developmental disabilities. Every service starts with the person: their needs, their goals and the life they want.
Our services at a glance
Residential (Shared Living)
In Shared Living, a person lives in the home of a home care provider and shares everyday life with them. It is a real home, with people who know them well.
The home care provider supports the person with:
- A healthy lifestyle
- Cooking and nutritious meals
- Daily living skills
- Making their own decisions
We focus on personal care and attention, a warm and welcoming home, and a sense of belonging. Our Vision for Independence team trains and supports every home care provider.
Respite
Caring for someone every day is rewarding, and it is also hard work. Respite gives families and home care providers a short break, while the person stays safe with a trained respite provider.
Respite can help with:
- Planned time off, such as a weekend away
- A family event, illness or other urgent need
- Regular breaks that help prevent burnout
Our respite providers follow the person's plan and routines, so the person feels at ease while their caregiver rests.
Crisis Response
Sometimes a person goes through a very hard time. It might be a sudden change in behavior, a mental health crisis or a home situation that is no longer working. When that happens, we act quickly to help keep everyone safe and steady.
Our crisis support includes:
- A quick response to the person, their family or home care provider
- Help to calm the situation and keep everyone safe
- Working closely with the person's team
- A follow-up plan to help prevent the next crisis
Community Participation Services (Day Program)
Everyone deserves a full and meaningful life in their community. Community Participation Services, often called a day program, give each person the support they need to get out, take part and be included.
Our support helps people to:
- Build independence and everyday skills
- Speak up for themselves
- Make friends and connect with other people
- Volunteer, take classes and enjoy local activities
- Use the resources and help available to them
Each person's schedule is built around their own interests and goals, and it changes as they grow.
Behavioral Specialists
Our behavioral specialists and Board Certified Behavior Analyst (BCBA) work with the person, their family and their home care provider. Together they look at what is behind a behavior and find positive ways to help.
What our behavioral specialists do:
- Get to know the person, their history and what matters to them
- Write a behavior support plan built around the person
- Train home care providers, staff and family to use the plan the same way
- Check in regularly and update the plan as things change
